Indicator Civil Registration and Vital Statistics
Definition Civil Registration is defined as the continuous, permanent and compulsory recording of the occurrences and characteristics of vital events, primarily for their value as legal documents and secondary for their usefulness as a source of statistics.
Death registration is the permanent and official recording of a person’s death.
Maternal death is the death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and the site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management, but not from accidental or incidental causes
Infant mortality is the death of an infant before his or her first birthday. The infant mortality rate is the number of infant deaths for every 1,000 live births.
Usual Residence refers to the place where the person habitually or permanently resides.
Place of Occurrence refers to the place where the vital event took place.
Municipal Form No. 103 (Certification of Death) is the prescribed form being used for the declaration of facts and circumstances surrounding the death of a person for purposes of registration.

Remarks Limitation Act No. 3753 otherwise known as the Civil Registration Law of 1930 mandates that all vital events shall be registered. Despite this, there is still a problem of under-registration of vital events primarily due to lack of awareness of the people on the importance of vital registration especially in remote areas.
